Before you schedule your exam...
Meade Optical participates with most insurance companies and plans. However, due to certain restrictions within counties, we are only able to accept certain plans in certain locations. Any patients with United Healthcare Community Plan coverage should schedule all of their appointments in the Watertown location, otherwise we will not be able to process your claim.
Please be aware that Meade Optical is not a Medicaid benefit provider. If you have any questions about your coverage or would like to make an appointment with our office directly, please give us a call at (315) 786-3937 in Watertown, or (315) 376-8159 in Lowville. We'd be happy to help!
All appointments made through the Online portal will be confirmed by phone call or text message 1-3 days prior to the appointment time. If you must reschedule or cancel your appointment for any reason, please do so as soon as possible so that the appointment slot may be used by another patient.
All examinations booked Online are the responsibility of the patient or guarantor. If you would like to have your exam billed to your vision or medical insurance, it is very important that you confirm your eligibility with your insurance company BEFORE making your appointment with our office. Most insurance cards will have a patient number to call to confirm eligibility, however if you would like assistance with this please call our office to speak with a receptionist.
Any examinations canceled less than 24 hours prior to their appointed time will incur a cancellation fee of $25 that will be charged directly to the patient or guarantor.
Medical Examinations are normally billed through a medical insurance company for the purpose of following up on another condition (history of diabetes, glaucoma, cataracts, red eyes, infections, etc.) If you have a history of any of these conditions please schedule this type of examination. Please note, these appointments are NOT emergency appointments. If you are having an emergency, please contact our office directly by phone or, if we're closed, please visit your nearest emergency room or urgent care center.
Vision Examinations are normally considered routine examinations not otherwise requested by another physician or for the evaluation of any specific problem other than to obtain a new prescription for eyeglasses or contact lenses. If you intend to have your examination paid for by a vision insurance (Davis Vision, EyeMed, Vision Service Plan, or March Vision for example, please schedule this type of examination.
* If you are an Established patient and do not already have a username and password from our receptionists, please do not register as a new patient - your previous exam information will not carry over to your newly created chart. Please call us and we will be happy to set up a username and password for you to access the online portal.
By clicking this button I hereby attest that I have read, understood , and will abide by the Patient portal policies and regulations outlined above.
